ACT is a comprehensive and integrated set of psychiatric, rehabilitation, case management and support services. Services are provided by a multi-disciplinary mental health treatment program mainly in your residence or other community locations.
A single site residence that provides group living for adults with 24 hour supervision/ staff, and rehabilitative activities.
A program which provides adults an apartment, in the community with visits as to provide rehabilitative services to improve function and develop greater independence.
A program which takes into consideration functional skills and the range of affordable housing options available and the extent of services and resources that recipients require to maintain their residence.
