Mobile Residential Support Team

Responsible for the training, oversight, monitoring and documentation of clients 
in self-administration of medication. Responsible for learning about medications 
and side effects.
Works collaboratively with treatment team, medical professionals, Health Home Care Coordinators and Housing staff to ensure client’s medical and health needs are met.
Assesses, evaluates and facilitates referrals to additional services as needed.
In the event of hospitalization, participates in discharge planning
Educates and trains clients on wellness and medication management.
Makes visits to assigned clients.
Participates with the clients in the development and evaluation of individual service plans.
Assists clients in establishing and improving in discussed goal areas. 
Provides crisis management and case management services. Includes making collateral contacts.
Provides Supportive Counseling.
Provides transportation as needed, for shopping, medical appointments, recreation activities, etc.
Notifies the appropriate personnel of any medical, psychiatric, or programmatic 

A Bachelor’s Degree in Human Services.

Demonstrates organizational and communication skills which would enable effective interaction with agency staff, consumers/clients/families and the community.

Critical thinking and sound judgment skills are necessary.

Valid Drivers License and available car 
Submits to Fingerprinting/Criminal background verification, as well as TB Testing
