Assertive Community Treatment (ACT)
Our ACT Teams are mobile, multidisciplinary programs that deliver wraparound behavioral health services to individuals with complex needs. Licensed by OMH, ACT is an evidence-based model designed for those whose symptoms significantly interfere with community living.
Each team includes mental health clinicians, nurses, peer specialists, prescribers, and other professionals offering:
Clinical assessment and diagnosis
Crisis intervention and stabilization
Medication management and health monitoring
Substance use treatment
Individual, group, and family therapy
Peer support
Vocational and educational support
Community re-integration services
These services are available in Nassau County & Suffolk County.


Mobile Residential Support (MRS)
Our MRS Team enhances care for residents in our Supportive Housing and Apartment Treatment Programs. The team delivers flexible, on-site and in-home services, including:
Medication management and education
Supportive counseling and crisis response
Health and wellness coaching
Peer recovery support
Linkage to medical and community resources
Our MRS Team is made up of nurses, peer specialists, medication coordinators, and other supportive professionals.
These services are available in Nassau County.
