Supportive Housing


Our Supportive Housing Programs provide permanent housing and support for individuals living with serious mental illness and substance use conditions, helping them integrate into the community. Funded by the New York State Office of Mental Health (OMH), Department of Housing and Urban Development (HUD), Nassau County Department of Mental Health, Chemical Dependency and Developmental Disabilities Services, and the New York State Office of Addiction Services and Supports (OASAS), these programs serve people who may also be homeless or transitioning from hospitals, state-operated residences, or psychiatric centers.
Residents contribute 30% of their income toward rent and receive rehabilitative and tenancy support services to achieve their personal goals and maintain stable housing.
These programs are available in Nassau County, Suffolk County, and New York City (Queens, Brooklyn, & Manhattan).
Apartment Treatment Program
Our Apartment Treatment Programs provide transitional housing and weekly rehabilitative services for individuals living with serious mental illness. These OMH-licensed programs help residents build skills, improve health, and prepare for more independent living environments.
Support includes counseling, life skills development, and goal-setting tailored to each individual’s recovery path.
These programs are available in Nassau County, Suffolk County, & Queens.


Congregate Treatment Program (Community Residences)
Our Community Residences provide 24/7 staff support in a structured setting for adults with serious mental illness. Licensed by OMH, these programs are transitional in nature and focus on restoring functioning, promoting stability, and developing skills for more independent living.
We operate four residences in Nassau County, including one specialized program for young adults ages 18–30.
These programs are available in Nassau County.
